Do not take iron solely because TIBC is low. Oral iron is appropriate when iron deficiency is demonstrated by the overall pattern, often including low ferritin, low saturation, and compatible CBC findings; many therapeutic products contain 45–65 mg elemental iron per tablet. Iron may be unhelpful or inappropriate when low TIBC results from inflammation, liver impairment, protein loss, or iron overload. Ask a clinician to review ferritin, transferrin saturation, hemoglobin, CRP, and liver tests first.
