Normālas TPMT rezultātu vērtības nenozīmē, ka azatioprīns ir drošs bez pastāvīgas CBC un citu drošības parametru uzraudzības. Saskaņā ar British Society of Gastroenterology IBD grafiku pārbaudes tiek veiktas 2., 4., 8. un 12. nedēļā, pēc tam vismaz reizi 3 mēnešos, lai gan citi protokoli var būt biežāki. Toksicitāte var rasties NUDT15 variantu, medikamentu mijiedarbības, ārstēšanas ietekmes vai ar gēniem nesaistītu mehānismu dēļ. Ja rodas jaunas drudža, mutes čūlu, sasitumu vai ievērojamu laboratorisko rādītāju pazemināšanās, jāveic agrīna klīniska izmeklēšana.
